Avery Telehealth Selected for CMS ACCESS Model

Avery Brings Its Digital Health Platform to Medicare Beneficiaries Through a Landmark Federal Initiative

Naples, FL—June 9, 2026 — Avery Telehealth, a digital health company specializing in remote patient monitoring (RPM) and patient engagement, today announced that the Centers for Medicare & Medicaid Services (CMS) has accepted its participation in the Advancing Chronic Care with Effective, Scalable Solutions (ACCESS) Model. ACCESS is a new 10-year CMS program expanding technology-supported care for Medicare beneficiaries with the goal of improving outcomes and reducing the total cost of care.

Avery will participate in the CMS ACCESS Model as a digital health partner to value-based care organizations and accountable care organizations (ACOs), providing technology, devices and education to support their patients and strengthen their success in value-based care. Avery’s acceptance across three clinical tracks, Early Cardio-Kidney-Metabolic (eCKM), Cardio-Kidney-Metabolic (CKM), and Behavioral Health (BH), positions the company to support the management of some of the most prevalent and costly chronic conditions in the Medicare population.

Avery will begin serving Medicare patients under the ACCESS Model in July 2026, bringing its remote monitoring and digital health programs to those managing hypertension, diabetes, heart disease, obesity, depression, and anxiety. Through its integrated platform and mobile app, Avery will manage patient enrollment and ongoing engagement, giving care teams real-time visibility into patient health. 

Avery will also provide care organizations with end-to-end ACCESS program management, including analytics, eligibility, and enrollment. ACOs and value-based care organizations will receive continuous data sharing and patient monitoring alerts, staying informed and connected to their patients’ care without additional operational lift.
