Avery Telehealth Partners with GuildNet to Provide Members with Chronic Conditions Telehealth Kits

MANHATTAN, NY., February 22, 2016- GuildNet, part of Lighthouse Guild, a leading non-profit vision + healthcare organization, with a long-standing heritage of addressing the needs of people who are blind or visually impaired has partnered with Avery Telehealth to enhance the ability of GuildNet members to live happy, healthy and at home. Avery Telehealth will provide GuildNet with telehealth kits to help members with heart failure, diabetes and/ or COPD to maintain independence. The telehealth kits will electronically send members vital sign data real-time to Avery Telehealth Nurses who will review critical alerts and be available to answer medical concerns or questions 24/7.

GuildNet will identify appropriate members for the new program that will be provided without co-pays and is free of charge to the member. It is projected 200 members with chronic conditions will be enrolled in the first 12 months.

“Through this collaborative partnership with Avery, GuildNet will be able to provide technology that could improve our members quality of life and reduce avoidable emergency room visits and hospitalizations,” says Wanda Figueroa-Kilroy, RN, MSN, President of GuildNet. “Eligible members with chronic conditions, like Diabetes, Heart Failure or COPD, will be able to provide real time clinical data that will enable us to — in collaboration with their physician— provide timely interventions.”

The key features of the telehealth program include:

  • Reviewing member’s medications to ensure they are working properly
  • Avoiding unplanned hospitalizations
  • Providing critical information to the member’s primary care physician so they can make informed decisions about their patients.
